Recently I asked for a copy of my tests results, because page 2 was missing. When I asked the nurse to find me the second page, she kept telling me that there was no second page, only the first and third page. After much begging, she finally got my folder out and found that there WAS a second page to the test result, but somehow she felt that her incompetency was my fault, and told me the next time I ask for a copy, she would have to charge me.
