My name is Patricia A. and in May of 2012 I became very ill. I could not move so one night I decided to call the ambulance and go to Weiss Hospital. Upon their examination they found out that I had a severe infection in my spine. So from there I had surgery and after that I had to have rehab. After about 3 or 4 weeks of rehab in Weiss, I transferred to Mid America Care Center. Upon arriving at Mid America Care Center I received the best of care. I was frightened coming here because I didn?t know anyone. The staff here welcomed me with open arms and tender care. Now it is September and I am ready to go home because of the extensive therapy I received at Mid America Care Center. So I say to anyone who needs to have the kind of care I had to have, please come to Mid America Care Center. They have excellent doctors, nurses, and rehab staff. Take it from me, you won?t find a better place. more

A while back I went to Jackson Park Hospital because my leg had become swollen and I could no longer walk. At Jackson Park Hospital, it was recommended to me that I go to rehabilitation center to continue my therapies. I chose a rehabilitation center that was close to my apartment and close to my friends on the south side of Chicago, which I can say was a mistake. I found the staff unprofessional and the facility unclean. I left the rehabilitation center and returned to my apartment but found myself back in the hospital about two and a half weeks later. I was in Cook County Hospital when again it was recommended to me that I continue my therapy in a rehabilitation center and my doctor felt that in my current condition, it would not be a good idea to live alone in my apartment. The doctor at Cook County Hospital recommended that I go the Mid America Healthcare and Rehabilitation Centre on the north side of Chicago. I had a bad taste in my mouth from my previous experience at a rehabilitation center and did not want to go through that again. I?m very glad I chose to come to Mid America. I found the staff to be very friendly and all the staff worked as a team. The facility is very clean and the staff were very attentive to my needs. The therapists worked hard to help me get strong enough to go back home. I?m hoping that I will not have to return to a rehabilitation center but if I do, I will come back to Mid America. Thank you to all the staff that helped me. more

For about the past year I was having trouble breathing and noticed I was becoming weaker. I had been to the hospital quite a few times and after each time I was discharged home. I was always going back to the hospital as I was having more and more problems breathing and doing the things I used to do every day. I also found it difficult when I was at home to keep track of my medications and sometimes I didn?t know what the medications were for. During my last stay in the hospital, it was recommended that I go to a rehabilitation center instead of going home again with a nurse who would come out and check on me. I had never been to a rehabilitation center and I did not know what to expect. On June 19, 2012 I came to the Mid America Care Center. I knew that in order to go home, I needed to get stronger. I knew that I had lost weight while I was in the hospital because most of my clothes hardly fit me anymore. I began working with the therapists and dietician who taught me how to exercise and helped me to get stronger. The dietician talked to me about what foods I liked and told me more about foods to eat that would give me energy. The nurses helped me learn about my medications and what the purposes of the medications were. I needed their help to teach me about taking medications and they gave me a lot of helpful tips so I won?t forget. After about 6 weeks of living at the Mid America Care Center, working with the therapists and other staff, I felt like my old self again. I gained about 10 lbs since being there and I felt much stronger than when I first got there. I would like to thank everyone at the hospital who helped me and for recommending that I go to the Mid America Care Center. I would like to thank my doctor and the staff at the Mid America Care Center for helping me get back on my feet again. I am anticipating going back home on July 24, 2012 and I will choose the Mid America Care Center again if I ever need to come back. more

I want to thank the Mid America Care Center for taking such good care of our uncle during his stay at your facility. Our uncle was admitted to the Mid America Care Center in December of 2011 from Weiss Hospital because he had become very weak, was having problems eating and was experiencing pain. Some months after he was admitted he was diagnosed with prostate cancer and we were told that the cancer had spread throughout his body. Our uncle passed away in June of 2012. The staff at the Mid America Care Center were very kind, knowledgeable, and answered all our questions about our uncle. Living out of the state made it very difficult for us to see our uncle. Though we were not able to see him as much as we would have liked, the staff helped us to speak on the phone with him on many occasions. I want to thank the staff at the Mid America Care Center and his doctors for making the last months of his life very comfortable. more

I started to have difficulty breathing and noticed that I was having trouble remembering if I had taken my medications. Around the middle of December I decided to take myself to Weiss Hospital where I was admitted. I???ve lived in my apartment on the north side of Chicago for over 20 years and was looking forward to going back there after I got out of the hospital. When the doctors and social workers told me that they wanted to send to a long term care facility I was immediately hesitant because I knew that meant I would not be returning home as soon as I thought. After thinking about what was best for me, I decided to go to a long term care facility. I knew that I wanted to be close to my apartment on the north side of Chicago and also wanted to continue seeing my doctor from Weiss Hospital. On January 3, 2012 I came to the Mid America Care Center. I was immediately impressed with the cleanliness of the facility and with how friendly the staff treated me. I found the assistance the nurses provided me to be very helpful and allowed me to stay on course with my medications. I was able to participate in physical and occupational therapy that allowed me to get stronger and taught me several exercises that I still use today. After staying at the Mid America Care Center for about 4 months, I was ready to return to my apartment. The Social Service team at Mid America Care Center helped me to arrange for home health nurses and aides to come see me in my apartment and to help me stay on course with my doctor???s appointments and medications. I cannot thank the staff at the Mid America Care Center enough for all that they have done. I would not hesitate to recommend them to anyone needing their services. more

Great THerapy!! 5/24/2012

I had been living in another long term care center in Chicago when I suffered an accident and went to Northwestern Memorial Hospital on March 21, 2012. I had some difficulties adjusting to the long term care center and knew that I wanted to recover at a different facility so I chose the Mid America Care Center. On April 5, 2012 I was admitted to the Mid America Care Center and was immediately impressed with the cleanliness of the building and friendliness of the staff. I was immediately started on physical therapy and began strengthening my legs and arms. I began receiving my dialysis treatments and was very happy to have the dialysis clinic located within the Mid America Care Center. After a few weeks of therapy I was ready to move on and am now planning on returning to Nicaragua with my family. I will be leaving the Mid America Care Center knowing that when I return to Chicago and need to the services of another long term care facility, I will choose the Mid America Care Center. Thank you for all the help you have given to me. more

While I was at my home, I had fallen and my wheelchair had tipped over on top of me. I think I was on the ground for what seemed like eight hours before my homemaker came in and found me. After she called the paramedics, they took me to Weiss Memorial Hospital where I was admitted and stayed there for about 5 days. The staff at the hospital explained to me that I would benefit from more therapy in a rehab facility and provided me with a few names of rehab facilities. I chose to come to the Mid America Care Center because it was close to my apartment and I had stayed there during a previous time after I had undergone hip surgery. On March 7, 2012 I was admitted to the Mid America Care Center and knew right away that I had made the right decision. I found the nursing staff to be very good and friendly. The food was remarkably good considering the number of people they have to feed. I found the therapy staff to be very well informed and taught me a lot about exercising and strengthening my muscles. It also helped me being close to home as I was able to go to my apartment and attend to some minor details. Overall I found the staff to be very friendly. I am looking forward to going home in the next few days and would not hesitate to come back to the Mid America Care Center if I ever need their services again. more

I wanted to take this opportunity to thank everyone at the Mid America Care Center for everything they have done for me. They assistance they gave me helped me to regain my strength after I suffered an accident. Around the middle of August 2011 I was walking down the street from my apartment when I tripped and fell on the sidewalk. I was in considerable pain and was taken by ambulance to the nearest hospital, which turned out to be Weiss Memorial Hospital. I was told in the hospital that I had suffered a dislocated left shoulder as well as a left Humeral fracture. I was experiencing considerable pain and discomfort while I was hospitalized. Once I was able to tolerate the pain, I began working in the hospital to regain my strength and coordination. Since my family lives in Puerto Rico and I was living by myself in an apartment on the north side of Chicago, I needed to go to a rehabilitation facility to continue my therapy. I chose to come to the Mid America Care Center because of its location, which was very close to my apartment. I came to the Mid America Care Center on August 31, 2011. I was very nervous about coming to a rehab center and did not know what to expect. I was very happy to find the people who work there were very friendly and helped me to adjust to my new living situation. I also found that many of the staff at the Mid America Care Center speaks Spanish so I was able to discuss my situation and any problems I experienced. I enjoyed working with the therapy staff at the Mid America Care Center who helped me to strengthen my shoulder and arms and taught me exercises that will help me stay strong after I return home. The staff at the Mid America Care Center also helped me in other ways that I cannot thank them enough for. They helped me to work with my building manager and took many trips with me to their office and the bank so I was able to pay my rent and keep my apartment. After my stay at the Mid America Care Center and help of the therapists in the rehabilitation department, my shoulder and arm is back to normal. I?d like to take this opportunity to thank everyone at the Mid America Care Center for all that they have done for me to help me get back onto the road to recovery?.. I started to experience trouble breathing when I was living at home and began contemplating how to best handle the situation. I thought about having oxygen at my home but then decided the best course would be to go to the hospital. I went to Northwestern Memorial Hospital around the middle of November 2011. I learned that I had fluid building up in my upper body and I started dialysis in the hospital to help with removing the fluid. After some time in the hospital, I was faced with the decision to return home with outpatient therapy or being admitted to a nursing home with in-house dialysis. I chose the Mid America Care Center because my wife had toured the facility. My wife had also toured other facilities with in-house dialysis. She found the Mid America Care Center to be impeccably clean and the staff to be extremely friendly. Based on her recommendation we chose the Mid America Care Center. I had serious reservations about coming to the Mid America Care Center or any nursing facility. The location was a principal factor as well as the in-house dialysis. I was admitted to the Mid America Care Center on December 6, 2011. I was immediately impressed with the attention to detail and caring attitude the staff showed me. I began receiving dialysis immediately and am very grateful to the dialysis technician for the expertise and assistance in explaining the ins and outs of dialysis. The nursing staff was very helpful and caring. The staff was receptive to my concerns and showed a great deal of compassion and understanding. The therapy team was very helpful and very patient in explaining the purpose of therapy. They gave me new insight into taking care of my body. They taught me numerous exercises that helped me regain strength as well as exercises I will use when I return home. Together with my team at the Mid America Care Center we planned for a date when I would return home. My discharge was planned with the help of the Social Service, Nursing and Rehabilitation Departments. I?m looking forward to getting back out there. If I had to return to a nursing facility, I would not hesitate to come back to the Mid America Care Center. more

I would like to take this opportunity to thank the many people from Schwab Hospital and the Mid America Care Center. Previously I had been diagnosed with breast and lung cancer. In the hospital, I had a bilateral mastectomy and went into respiratory failure requiring a breathing tube. I also had a feeding tube inserted because I had a lot of difficulty swallowing my food. On July 21, 2011 I was admitted to the Mid America Care Center to continue my rehabilitation that I had started in the hospital. I immediately felt at home when I arrived. The staff at Mid America took their time with me in explaining how the care would be handled and were eager to start working with me. The nursing staff took very good care of me and helped me to heal 2 skin wounds and the surgical scars from my mastectomy. They were very patient with my family and me and explained everything they were doing for me. The rehab team did a very good job working with me on exercises and helped to regain a lot of the strength I lost. Since I still was having trouble swallowing, I still had the feeding tube that helped me gain additional nutrition. I got stronger and healthier as I kept working with the staff. I started to talk with my family and care team about going home which everyone thought was a very good goal for me. About 2 weeks ago, I had my feeding tube removed and at last I felt like I was finally ready. With the help of the staff at the Mid America Care Center, I?m looking forward to living with my family again. I?ll have the help of a nurse who will come to my house to assist my family with my medications and to give some additional help. I want to thank everyone at the Mid America Care Center for all that they?ve done for me. I wouldn?t have been able to recover without their help. more

I had been having problems with my leg for about the past 4 ? years. I had been going to my doctor for several years when they recommended that I have surgery on my leg. I made my surgical appointment with the surgeon at Weiss Hospital around the beginning of October 2011. Following my surgery, I was kept in Weiss Hospital for a few days until they recommended that I continue my therapy and rehabilitation at the Mid America Care Center. In July 2010, I was assaulted by a person unknown to me while I was walking down the street. I suffered multiple broken bones and was hospitalized. I chose to be admitted to the Mid America Care Center because several members of my church were living or had at one time lived at the Mid America Care Center. After rehabbing my arm, I stayed at the Mid America Care Center for about a month and a half. Around the 1st week of August of 2011, I was working for a customer of mine and was removing broken or dead tree limbs from the roof of his house. The ladder I was using was leaning against a tree limb that broke away. I fell about 20 feet to the ground and fractured 2 vertebrae in my back and the tree branch that broke hit me in my mouth and knocked out the upper part of my teeth. I was taken to Illinois Masonic Medical Center where I was hospitalized for a little over a week. I began my therapy in the hospital and had to decide where I was going to continue my therapy. As I was ending my stay in the Illinois Masonic Medical Center, the hospital staff told me they would be sending me to a rehabilitation facility to continue my therapy. I knew based on my previous experience that I wanted to continue my therapy at the Mid America Care Center. On August 13, 2011 I was admitted to the Mid America Care Center. While staying at the Mid America Care Center, I found the nurses to be extremely helpful and explained numerous things to me that I found very useful. On one specific occasion I was having difficulty with back pain. My nurses discussed my situation with my doctor and found pain medication that was more effective than what I had been taking. The therapy team worked very hard to get me back to my highest level of functioning. They were patient with me and provided positive reinforcement. I also found the nursing assistants are very polite and caring, not just to myself, but also the other residents I witnessed them taking care of. Walter D. 9/2011 When I was interviewed about coming to Mid America from my bed at Thorek Hospital I was very apprehensive about even a short-term stay in a nursing home. My previous experience with a nursing home was my mom?s boyfriend being confined to a really unclean and understaffed one on the south side of Chicago. It isn?t often in life that you anticipate a bad experience and find not just a good one but also a spectacular one. First of all the staff is to be commended for not just their extreme professionalism but for the kind, beautiful and caring attitude for everyone under their care. It is a revelation! Then the facility itself is so immaculately clean. I have frankly never seen any business even surmount hospitals I?ve visited to be so constantly and consistently maintained. I am writing to explain my situation and the experiences I encountered while residing at the Mid America Care Center. Prior to being admitted to the Mid America Care Center I suffered severe medical complications while living in my apartment near Midway Airport. While in the hospital, my medical condition stabilized but I still had a long road to recovery ahead of me. I had a feeding tube inserted into my stomach because I was not able to successfully swallow food or water. I came to the Mid America Care Center on February 22, 201 l. When I was admitted, I still had my feeding tube but I started working with the Speech Therapist. Not long after working with the Speech Therapist, I began to have more success swallowing food and water without coughing. The facility staff also assisted me with my meal times and before long l was feeding myself without any dif?culty. On June 27, 201 l I had my feeding tube removed. Working with the therapy team helped me regain the independence I experienced prior to becoming sick. I worked very hard with the therapy team who helped rebuild my strength and gave me constant encouragement not to give up on the plan of being able to walk again without any assistive devices. After many days and hours of working with the therapy team, I?m now able to dress myself, shower independently, and no longer use a wheelchair or walker to get around. The Mid America staff on the 2""? floor provided excellent care from the time I was admitted until I left. From helping to manage my medications or just giving their ear to listen to me, the staff was always patient and understanding to my needs.
