I suggest you stay away form this Dental Office. They are running scam wherein they will collect more money from you as your out of pockets expenses. I went there few months ago. Paid the agreed Out-of pocket fee then a week later, I got call saying I need to pay more. Bewareif you dedide to go to this Hell hole. Review all the documents that you are signing. ALL of these documents are to protec their rights and interests, NOT yours.
