Took my 2 girls for annual physicals. Upon completion of physical, asked nurse for a copy of each child's 1 page well being exam (weight, height, vision, etc. recorded). My request was refused; she stated that I had to request a copy of these 2 pages through medical records and pay for them! Moreover, when Dr. Eibschutz examined my daughters, I asked her to sign and date my daughter's one-page form for preschool. Again, Dr. Eibschutz refused my request stating that I need to pay an additional $10 for her to do this, and that I needed to ""respect Summit Medical Group policy."" I felt disrespected as a mother and devalued as a patient and client. I paid $40 copay, my insurance company, United Health Care, was charged hundreds of dollars for this visit, and I was neither given my children's medical records (2 pages) or had my form competed (1 page). This is the height of outrageousness. Summit Medical Group is clearly all about the American dollar and not about patient care or common kindness. I will not return to this doctor or this clinic.
