I walked into this office and was pleasantly greeted; I decided to make an appointment the next day. I explained to the receptionist that I will fill out the paperwork ahead of time since I will be attending appt with my baby. She neglected to send me the forms. At 10:35 - I am then presented with a pad to go through the forms, and still have to wait to be seen! An explanation of ""they are having problems with emailing the forms"" was given to me. A phone call should have been made to be this morning before leaving my home with my child that I should come in at least 30 minutes prior to fill out paperwork that should have been given to me a day prior.
