Scheduled cataract surgery with Dr. Rusinek. At pre-op appointment, did not see him, but a nurse took all the measurements, etc. She was pleasant, but she called the wrong prescription (allergic) into the wrong pharmacy (I had to contact the pharmacy and have it transferred), and signed a form as a witness without first ""witnessing"" the signature, which never took place -- had to drive back to office to pick up and sign. Pre-Operative Cataract Instructions conflict with the actual instructions on two of the three prescriptions. An awful lot of of cleaning up behind some sloppy work. Not impressed - hopefully not representative of rest of staff and Drs.
