Unattentive to care needs. Nurse call button took an average of 15-30 mintues. One call was never answered and had to have an outside person call the nurses station to get someone to respond to the patient's call. Patient fell out of bed and was not responded to for over 20 minutes. Incident report was inaccurate and after four requests and a care meeting, the report is still inaccurate. On several occassions visitors located the call button attached to the underside of the foot of the bed when patient could not locate it. Meds were not being administered according to perscription instructions. Patient was not aware of what was being taken. Personal care equipment was damaged by staff and patient clothing is missing (not out to be washed). We will be filing a formal complaint with the state.
