Review content:
Candance, from the billing department (I assume), refused to assist me (or even speak to me calmly) when I needed to determine what went wrong on a medical claim they submitted to my insurance. The hard copy of the form that is on file with my insurance company did not match the form that Candace stated she had on file. When I asked for a copy of the form, she said she needed written approval from my husband, even though I'm the primary card holder (which I later found out they submitted that information incorrectly too). Fine! So my husband called, got it faxed to him and then faxed it onto me. Among other mistakes (as previously mentioned), the portion of the form in question looked a bit odd, almost like it had been tampered with. Regardless, it was ""submitted"" to me just how I needed it submitted to the insurance company. So, I called her back to ask her to simply resubmit it. I had to hear her go on and on about how that's not the way it works. So, I had another raised voice discussion with her, where she stated she couldn't help me. Thanks for nothing.
