Enclosed is a Patient Information form as well as a Visual and Medical History form. Please bring these two completed forms with you to your appointment. This helps improve the office flow which decreases appointment wait times. A bio graphy page has been included to introduce you to the staff and the doctor. As you can see, our office offers over 20 years of experience in vision care. Finally, we have included some informative brochures which illustrate some of the latest technology in eyeglass lenses and contact lenses available today.

Dr. Brett Bowman is a 1994 graduate of the Southern California College of optometry. Dr. Bowman then completed a hospital-based ocular disease residency with the Indian Health Service. He has lectured to physicians on the visual effects of diabetic retinopathy,and has been published in the prestigious Journal of Optometry on ocular migraine. Dr. Bowman is board certified in the use of diagnostic and therapeutic drugs and is capable of treating many eye diseases such as pink eye,allergic conjunctivitis,and dry eye. He married a fellow optometrist,Dr. Laurel Jensen,and has two young children. In his spare time,Dr. Bowman enjoys camping,skiing,and coaching his kid's sports teams.
