Transitions was founded in 1955 as the Adams County Mental Health Center. For over 50 years, the agency has served the Adams County area, providing high-quality, affordable mental health and rehabilitation services.
Transitions is a United Way agency and is CARF (Commission for the Accreditation of Rehabilitation Facilities) accredited. We have undergone a rigorous survey process to have attained CARF accreditation.
