When you arrive at Henry County Hospital, one of our registrars will obtain necessary personal and medical information. Additional information will need to be taken as you are admitted to your room. Much of this is required by law or by your insurance company.
Your information is kept in strict confidence and is part of your medical record.
A Consent to Treatment form must be signed by you or a responsible party at the time of your registration. This form authorizes us to submit your bill and any required records to your insurance company, and it indicates your acceptance of financial responsibility for charges not covered by your insurance.
Forms for minors must be signed by a parent or guardian. You also may be asked to sign additional consent forms as need the arises.
While you are a patient at Henry County Hospital, your nurses will want to keep your family members/patient representative informed of your health status. While we realize it is very important to communicate information regarding your care to your family, we also recognize we have an obligation to you, our patient, to protect your privacy.
