I bought the smile choice 200 which costs $19.11. The copayment schedule price list is very attractive. For example, a silver amalgam filling only costs $8 for one surface.
However, shady dentists like to add random charges, they will tell you that this thing is not covered by your plan, that thing is not covered by your plan. Even if they use the same American Dental Association procedure code which are listed on your copayment schedule, those shady dentists still add random charges. When you ask them why, they have all kinds of reasons, such as the medicine are not covered, or fees are different for cosmetic or erosion of the teeth.
Even though Smile Choice plan 200 offers free prophylaxis cleaning, dentists won't do it even if you specifically request for it. They want you to get the deep cleaning (periodontal scaling and root planning) which costs $30 per quad.
And this company can't enforce the price with those dentists. So in the end, even though you have the plan, you are not gonna save much money because the dentists still want you to pay more unless you can actually find an honest dentist which is hard.
