St. Joseph's/Candler's Center for Medication Management has five convenient locations to help you with your medication needs and concerns. Our primary objective is to assist you in the better understanding of your medications through disease state education and knowledge, proper use and administration of the medication, monitoring parameters, and specific therapy related goals. Our ultimate goal is to improve the standard of care with regard to medications, to reduce adverse side effects and drug interactions, improve the coordination of care among healthcare professionals, and reduce hospital readmissions.
