Repeated acts of clearly excessive prescribing, furnishing, dispensing, or administering of drugs or treatments, repeated acts of clearly excessive use of diagnostic procedures, or repeated acts of clearly excessive use of diagnostic or treatment facilities as determined by the standard of the community of licensees is unprofessional conduct for a physical therapist. a. Patient was treated for 127 visits of physical therapy without a re-evaluation to support the continuation of manual therapy and daily exercise for three years b. Patientâ??s lack of progress of her chronic neuromuscular problems which were not taken into account when the same physical therapy treatment was continued. Specific examples where the same treatments were continued when no longer appropriate can be found in daily notes and exercise logs for patient from April 2, 2004 through June 18, 2004. c. Patient record indicated there is no evidence to support the continuation of physical therapy care beyond 29 visits during July 30, 2001 through March 8, 2002. California Code of Regulations Section 1399.85 A physical therapist shall document in the patient record the following: Examination and re-examination Evaluation, when the patient is to be reevaluated and the reevaluation Diagnosis Prognosis and intervention Treatment plan and modification of the plan of care Each treatment provided Discharge Summary Business and Professions Code Section 2620.7 A physical therapist shall document his or her evaluation, goals, treatment plan, and summary of treatment in the patient record A physical therapist shall document the care actually provided to a patient in the patient record. I'm an Indian. been to many Indian restaurants. But this is the WORST. Please dont come to a conclusion on Indian foods based on this restaurant. I have thrown away part of my 'To-go' food ... I really dont know why they have a tag 'Indian restaurant' Pros: Packing Cons: All others more

the food may be decent. but don't ever ever, ever CATER from this restaurant. They suck!!! Their customer service sucks. They sweet talk you and say they are honest people, and that they will give you enough food. But they didn't. I catered a party of 35 people. They gave me food for 23 people. We were embarrassed. WE had to call another restaurant and cater additional food for a dozen people immediately. Never cater from them. You have been warned. Worst experience. When I called the same day to talk to the owner/manager regarding this issue, 3 times the receptionist told me he'd call me back...he didn't!! When Sarwan (the owner/manager) did call back, he said it's our fault. He called us Liars, and that he wouldn't reimburse or compensate us in any way. Not even food compensation....very cheap!!! My wife and I love Indian food and although we are not ethnically Indian and would appreciate a home cooked Indian meal, this lunch buffett is very good by South Bay standards. It is more flavorful, with more variety, at the lowest cost of any other place. Very courteous and attentive staff. Decent Chicken Tandoori. Would like to try this place for dinner to get a better appreciation of their culinary expertise. someone told me about kallencal's review and i couldn't believe it. this is a wonderful, caring group. they helped me after i had a car accident with a head injury. polly was totally kind, professional, caring, and healing. she was able to provide very reasonable fees and helped me enormously. i saw her on a regular basis for about a year, after which i've seen her on and off for other issues. she is a wonderful therapist, has great staff, and provides a comfortable, safe environment. she helped me enormously with my injuries. Pros: caring, professional therapists and office staff. clean, comfortable environment. the best! more

i have had a lower back problem for many years and have been to many practitioners-- chiropractors, p.t.s, massage therapists, etc., and this group was the only one that made substantial inroads into my healing. the place is clean, organized, friendly, and welcoming. they are very reasonable in cost-- in fact, they charged less than many of the competitors. they are highly professional and i would recommend them. have told friends who have also had the same experience with body rehab center. you'd be in good hands to go here. more
